Parental Consent for Use of Name, Likeness, and Media

I hereby grant permission to Forensics Forever to use my child's name, likeness, photographic image, and/or video recordings in any media or format, including but not limited to TV programs, social media platforms, websites, brochures, promotional materials, newspapers, as well as other online and print publications. I understand that if, at any time, I choose to revoke this permission, I must notify Forensics Forever in writing. Upon receipt of my written notice, Forensics Forever will cease using my child's name, likeness, and/or image in future materials, and will remove any references or images from its webpages and social media channels within sixty (60) days. I acknowledge that Forensics Forever cannot control or be held responsible for any use or downloads of my child's likeness made by third parties prior to its removal from the internet. Furthermore, I understand that any previously printed or distributed publications may continue to include my child's name, likeness, and/or image even after revocation.

Assumption of risk, release and waiver of liability and indemnity agreement

I affirm that my child and I are physically able to participate in Forensics Forever activities. I acknowledge that Forensics Forever does not provide health or disability insurance, so I accept full responsibility for our coverage. I understand there are inherent risks involved and confirm my child is in good health and fit to participate. In a medical emergency, Forensics Forever will call 911 but assumes no responsibility for treatment or expenses. I hereby release and discharge Forensics Forever, its officers, employees, volunteers, and agents from any liability for injury, death, or property damage connected to these activities, whether caused by my negligence or otherwise. I also agree to indemnify and hold harmless Forensics Forever and its representatives against any legal actions or costs arising from my participation or my child’s participation.

Cancellation Policy

I acknowledge that when I register my child for courses, camps, clubs etc at Forensics Forever and pay the fee, there are no tuition refunds once the program has started. If requested before the program has began, a full refunded will be granted minus $75 administrative fee. I understand that it is my child's and mine's responsibility to arrive for class or log in online on time and be prepared with all the necessary material and completed assignments. If a student misses a class it is their's and the parent's responsibility to make sure all of the work for the next class is complete or ask for a makeup class when applicable. Students and Parents agree to comply with all of Forensics Forever's policies and procedures stated here and introduced during the period that the child participates in the program. Forensics Forever staff and program directors hold the right to deny any student the right to attend classes if the said student's behavior does not comply with Forensics Forever's policies and procedures.

Temporary Shift to Online Instruction

At Forensics Forever, we prioritize student safety and continuity of learning. In the event of unforeseen circumstances such as a teacher's contagious illness, lack of substitute availability, or environmental factors like poor weather conditions, we reserve the right to temporarily move in-person classes to an online format.

We will notify families as early as possible and provide clear instructions for accessing the virtual class. These transitions are made only when necessary and are intended to ensure that students do not miss valuable instructional time.

By enrolling in our programs, you acknowledge and agree to this potential shift when circumstances require it.
